How:
Gather completed documentation:
1) EOP Signature Page (be sure candidate contact information is current)
2) Copy of original Individualized Professional Development Plan (IPDP) from Beginning of Plan (BOP)
3) Documentation of completed activities for original IPDP
a. Documentation from mentors must be on employer letterhead and include all items that require the mentor's approval
b. Copies of transcripts for coursework can be included for employer review, however official transcripts with registrar’s signature must be uploaded by candidate into Educator Information System (EIS) account.

Once EOP Signature Page is completed, create a single PDF document of the documents collated in the order listed above to provide to your employer following local procedures.

EOP submission PDF files must be less than 15 MB. We suggest scanning black and white, at 300 dpi resolution. If the document is already a PDF and you can use Adobe Acrobat Pro, you can reduce the PDF size - Menu/Save as other/Reduce size PDF:

If you have already held a NH educator license, which is currently expired, there are additional steps needed to apply for a new endorsement. Please use the instructions to Obtain a New Endorsement with an Expired License.

The following roles in EIS are able to upload plans: Superintendent, Superintendent Designee, Charter School Administrator, Non-Public School Administrator. These roles will be able to submit the EOP electronically through the EIS/SBLP/EOP Submissions tab. EIS: Status Updates

EOP’s submitted by the candidate will not be accepted.

EOP Review

How:
Once an EOP is uploaded, the candidate will receive an email on how to submit payment. This must be done within 5 days of the EOP submission. Once the fee is received, the plan appears on an evaluator’s queue and will be reviewed for completeness.

The plan will either be accepted or a revision requested. The candidate and employer will receive an email response in both scenarios.

The plan will be “sent back” electronically through the EIS SBLP tab and the employer will have 10 days to upload a new revised single PDF.

EOP Approved

How:
