Collection of SAU and School Contact Information

On an annual basis, school districts must confirm or update their contact information for the district and school(s) through the A12G form, which is available in the Education Statistics System (ESS) within the myNHDOE Single Sign-On System. The district i4see coordinator (a.k.a. Local Security Administrator) completes this form.

The Assessment Team uses this contact information to send important updates to the following test coordinators listed on the form. Please ensure that the contact information is accurate for the current school year.

• DLM Test Coordinator

• NH SAS Test Coordinator

• SAT Test Coordinator
