Test Evaluation Request

Test evaluations are only available for students enrolling or enrolled in a NH approved educator preparation program that are not yet eligible to apply for licensure. All other applicants must submit test score reports with an application for evaluation.

All test evaluations must be submitted online following the directions below. Test evaluation forms are no longer accepted. You will be sent an email with the results of your test evaluation. The email and your test results can also be viewed in your myNHDOE/EIS/View Your Credentials/Emails and Tests sections.

If you already have a myNHDOE account and an Educator Information System (EIS) account, please log into myNHDOE/EIS/Educator and start at step 12.

If you already have a myNHDOE account, but do not have an Educator Information System (EIS) account, please log into myNHDOE and start at step 5.

If you do not have a myNHDOE account, start at step 1.

 

Report any error messages you encounter to our Help Desk and await further instructions. Do not attempt to make multiple accounts.

 

In your browser go to https://my.doe.nh.gov which is the required State of New Hampshire single sign-on portal to access a variety of State systems. Click on “Create an Account”

Fill in the required information and then click “Next”.

NOTE: If you use a work or school email address we strongly advise you to include a personal email address as an alternate.

Create a Username and Password. Record these in a secure location and never share your login credentials with another person. Provide information that can be used to recover account access should you forget your login credentials. Click “Next”.

You then come to the confirmation screen. Click Enter to finalize the Create User Account process.

This brings you to the myNHDOE Systems screen where you will see a “New Applicant” option for the Educator Information System (EIS). Click on the New Applicant link to enter EIS at this time.

You will land on your personal Home Page in the Educator Information System with only one option – Create Profile. Click on that link at this time to create your file and generate an Education ID Number (EdID).

Create an account 1 of 4: you will need to enter some basic information. Fill in the information and then click “Next” to continue.

NOTE: the experience questions refer to the number of years of experience as an educator in a public school and/or a private school in New Hampshire (in-state) or another jurisdiction (out-of-state). Because these fields are required you should enter 0 if you have no experience of the type described.

Create an Account 2 of 4 – this next step asks for your mailing address. Be sure to double check spelling.

Create an Account 3 of 4 – the system next asks for telephone and email contact information. We strongly advise that you enter both a primary and alternate telephone number and email address. Please be advised that email addresses are used to recover an EIS profile if you forget your login information and school/employer email addresses are subject to change beyond your control.

Create an Account 4 of 4 - a confirmation that the Create a Profile process is complete. Please make note of your EdID number as this will be your permanent unique identifier. Click Submit.

The system will then display your EIS landing page. This is the same page that will be displayed each time you log into EIS using your Educator role. If you are not submitting a Test Evaluation request you may stop here and log out of EIS using the link on the left side of the screen.

It is from your Home Page screen that you will begin the Test Evaluation Request process. Click “Online Applications”.

On the Applications and Evaluations page click on the Test Evaluation link at the top of the list.

Test Evaluation Request Steps 1 - 3 ask you to verify the personal information in your profile.

Criminal History Disclosure Statement:

  1. Includes ‘charged pending disposition’.

  2. Includes all ‘educator’ credentials. Revoked either permanently or temporarily (might be referred to as ‘suspended’). This question includes NH as well as any other state or jurisdiction.

  3. ‘Surrendered’ means given up by the credential holder before the scheduled expiration date, generally for reasons of misconduct. It also includes credentials that were allowed to expire and not renewed due to questions of conduct. ‘Surrendered’ does not mean a credential that has expired (sometimes called ‘lapsed’) on a standard schedule and was not renewed because the credential holder moved to another state, had retired, or for some similar reason.

  4. Refers to any investigation that could result in suspension or revocation of an educator credential.

Use the first dropdown box to select the institution that you are, or intend to, attend. Then use the second dropdown box to select the educator preparation program in which you are interested.

Click “Browse” and navigate to the applicable score report file on your computer. This must be a PDF document. Double click on the file name to add it to the wizard screen. The “Document Type” is pre-set for you.

Once you have added a score report you are presented with a button to open and view the document in order to verify that you have attached the correct score report. If you have attached the wrong score report you have the ability to “Remove” the document and to replace it with the correct one. At this point you may either click “Browse” to attach additional score reports or “Next” to save the documents to your EIS file.

The final step in the Test Evaluation Request process is to verify that you are done uploading score reports. Make sure the radio button is checked and click “Submit”. When you land back on your EIS home page click “Logout of EIS” and you are done.
