School Nurse DOE Renewal

Complete the Renewal Form listing Dates, Number of hours completed, and the Title of activity and Sponsoring Organization, and sign form. Applications submitted for renewal may be selected for audit. If you are audited you will be requested to provide verification of completion of the professional development listed on the renewal form.

Professional growth is the acquisition of additional information or skill related to one's role as a School Nurse. It consists of such activities as college courses, local in-service seminars or workshops, institutes, independent study and research. This includes continuing education units (CEU's) to renew your RN license.

A minimum of forty-five (45) continuing educational units of professional growth related to school nursing is required during the three years preceding the date of application for recertification. Participation in growth activities must be documented AND verified. One continuing educational unit equates to one clock hour.

SCHOOL NURSE I IMPORTANT INFORMATION

  1. A school nurse I license can be renewed one time with verification of enrollment in a board of nursing
    approved ADN to BSN program that shows a completion date on or before the expiration date of the school nurse I renewal license.

  2. A school nurse I shall be assigned a mentor licensed as a school nurse II or III; be appointed by the
    superintendent; and have at least 5 years of experience as a school nurse.
