The New Hampshire Department of Education’s Division of Educator Support and Higher Education Bureau of Credentialing office has been relocated to 25 Hall Street, Concord, NH 03301.
The forms below include an outdated mailing address. All USPS and carrier deliveries (FedEx, UPS, etc) must be addressed to 25 Hall Street, 3rd Floor, Concord, NH 03301. Forms will be updated here as soon as the Administrative Rule approval process is completed.
